Gangrene of the middle finger

51 y male presented with puss and gangrene of middle finger of left hand . The condition started about 13 days ago after the iron piece fell onto his hand and cut the skin deeply. It was sutured then And irrigated. But it didn't progress to healing very well. Yesterday he applied sugar and soap and covered the wound. O/E : the gangrene is starting from distal phalanx at the inner aspect of hand preserving outer aspect , the wound irrigated and its deep and you can see the nerve Fbs:150 HbA1C:5.8% Whats your lines of management?
